Social Worker – Mental Health Hospital Discharge Team
Location: Colindale, London
Rate: £32.20 per hour (Umbrella)
Contract: Locum
We are recruiting for an experienced Social Worker to join a busy Mental Health Hospital Discharge Team in Colindale.
This is an excellent opportunity to work within a multidisciplinary setting, supporting adults through safe and effective hospital discharge pathways while promoting independence, wellbeing, and positive outcomes.
Key Responsibilities
* Undertake Care Act assessments, reviews, and support planning.
* Complete Mental Capacity Assessments and safeguarding enquiries.
* Manage a varied caseload within a hospital discharge setting.
* Work collaboratively with health professionals, community services, and partner agencies.
* Develop strengths-based, person-centred care plans to support adults and carers.
Requirements
* Qualified Social Worker with current Social Work England registration.
* Experience within Adult Social Care, Mental Health, or Hospital Discharge services.
* Strong knowledge of the Care Act, Mental Capacity Act, and Safeguarding Adults procedures.
* Excellent assessment, communication, and case management skills.
