Care Coordinator (with Health Care Assistant Duties)
The Care Coordinator will play an important role within thePrimary Care Network (PCN), working as part of the multidisciplinary team todeliver high-quality, patient-centred care.
The primary focus of this role is to provide clinical support as aHealth Care Assistant (HCA) undertaking tasks such as blood pressuremonitoring, blood tests, ECGs, and other routine procedures.
In addition, as part of their role, the post holder will providesecondary support to the Care Coordination team, assisting with administrativetasks, reception cover, and rota coordination to ensure smooth day-to-dayoperations across the PCN.
This hybrid role requiresexcellent clinical competence, attention to detail, and strong communicationand customer care skills.
Main duties of the job
Clinical(HCA) Duties
* Perform routine clinical observationsincluding blood pressure, pulse, height, weight, and BMI.
* Undertake phlebotomy (blood taking) andrecord results accurately.
* Carry out ECGs and ensure documentationis completed appropriately.
* Support clinicians with patientpreparation, specimen collection, and clinical documentation
* Ensure clinical areas are maintained toinfection control and safety standards.
* Record and update patient informationaccurately on clinical systems in accordance with policy
* Support health promotion and basic patienteducation under supervision.
Administrative& Care Coordination Support
* Process and effectively signpostpatients to the appropriate healthcare professional, depending on thepresenting condition
* Answer incoming phone calls,transferring calls or dealing with callers requests appropriately.
* Process patient requests forappointments and repeat prescriptions in line with practice protocols.
* Initiate contact with and respond torequests from patients, team members, and external agencies.
* Enter Read/SNOMED CT coded data
* Assist with rota coordination withinthe reception and Care Coordination teams.
* Provide support to practices with MDTmeetings and general coordination activities.
* Carry out data entry, system searches,and reporting tasks as requested.
* Support clinical and administrative staff withgeneral tasks as required

The St Helens South Primary Care Network Limited Company (PCN) isthe organisation that delivers the core components of the PCN Contract as wellas supports the wider Primary Care system and tenders for, designs and developsnew/ existing services for the benefit of the St Helens South population.
The PCN is comprised of 10 Practices in the St Helens South areas.The practices have a combined geographic area made up of around 85,000patients.
This role will contribute to the improvingquality of care of our patients across the PCN and its network of practices.

Person Specification
Experience
* Experience working as a Health Care Assistant or in a patient-facing healthcare role
* Experience providing administrative or reception support
* Previous experience in a PCN or GP practice setting
* Experience with rota coordination or supporting MDTs
Qualifications
* NVQ Level 2/3 in Health and Social Care or equivalent
* Evidence of ongoing professional development
* Phlebotomy and ECG training / Qualifications
* Customer service or administrative training
