Per-Visit Rates
$75 Per point pay
$75 per regular OT Visit
$112.50 per OT Eval
$187.50 per OT SOC

Job Description
* Evaluate patients and perform appropriate therapy interventions
* Develop a comprehensive plan of care and coordinate multi-disciplinary care
* Perform prompt clinical documentation and visit notes
* Continued communication with physician, care team members, patient and family
* Comfortable working in all aspects of the home health setting
* Manage patient treatment and outcomes directly, or through COTA supervision
Requirements
* Strong organization skills and time management
* Able to multitask and accomplish and meet timelines
* Professional appearance and effective communication skills
* Current Texas Occupational Therapist (OT) license
* One year of occupational therapy experience
* Current Texas driver's license
* CPR certification
* Experience with healthcare software (Homecare Homebase preferred)
* Previous Home Health experience is helpful, but not required
