Job Summary

We are looking for an Advanced Nurse Practitioner, ANP to act autonomously within their professional scope of practice providing care for patients, face to face, telephone consultations and home visits.
Main duties of the job
The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers, and colleagues, ensuring clinical safety and excellence.
The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients, making any necessary referrals for investigations in the appropriate manner.
The postholder will use skills, knowledge, and competencies as a qualified Advanced Nurse Practitioner to be responsible for the ongoing management of patients.
About Us
We have a team of supportive GPs who enjoy their job and who value traditional family medicine and continuity of care.
We have a clinical team consisting of 4 GP partners,1 ANP specialising in general practice and two prescribing nurse practitioners who specialise in diabetes and chronic disease management.
Exceptional administration and management team who work hard to support the clinicians and reduce workload.

Advanced Clinical Practice
* To demonstrate advanced knowledge and skills and clinical reasoning to deal with complex cases within their specific field.
* Independently adjust treatment plans according to need assessing appropriate management options, and initiate treatment or referral to an appropriate professional where necessary.
* To demonstrate advanced critical thinking and analytical skills to support clinical judgment and decision making process.
* To carry a clinical caseload providing advice and guidance to other staff on complex cases.
* To be recognised as a highly skilled clinician within area of speciality, using a wide range of highly specialised and evidence based therapeutic interventions, treatment techniques, and modalities.
* To carry out comprehensive assessments of patients as an autonomous practitioner, particularly those with highly complex presentations.
* To keep comprehensive, accurate records which support the clinical reasoning process in the assessment and treatment of all patients, which meet departmental, Health Board and Professional standards.
