The Frailty Service is a multi-disciplinary service within the Primary Care and Community Division, centred on providing people with care and/or treatment close to home, promoting independence via reablement teams. The aim of the service is to reduce unnecessary hospital admission by providing safe alternate pathways and minimising hospital stay by facilitating early and safe discharge. The redesign of services for older and/or frail people means that you will be joining the Primary Care and Community Services Division at a once in a generation era, helping to develop a 'best in class' Frailty/Intermediate Care service model.
