Health Care Assistant

We are looking for an enthusiastic and skilled Health Care Assistant to join our clinical team at The Saltaire & Windhill Medical Partnership. Ideally you will be qualified and experienced in delivering phlebotomy, long term condition reviews, wound care and assisting with minor surgery and contraceptive procedures. We will consider applications for full time or part time hours.
Main duties of the job
ECG Recording, Phlebotomy, LTC reviews, assisting with diabetes clinics, injections under PSD, vaccine clinics, testing of urine samples as requested by clinicians, diabetic foot checks, NHS Health Checks, asthma reviews (subject to training, Wound Care, BP checks, chaperoning duties, processing and management of lab samples requested by GPs and Nurses, Vaccine and cold chain storage, monitoring and recording, stock control and stocking rooms, assisting with minor surgery and contraceptive services, administration as necessary.

The Saltaire & Windhill Medical Partnership is a four site practice based in Saltaire, Shipley, Baildon and Cottingley. We have over a 100 staff providing patient services to a patient population of 24,500.
Job responsibilities
JobSummary
Tosupport the practice team in the delivery of nursing services, working as partof the practice multidisciplinary team, delivering care within specificpractice guidelines and protocols. Thehealth care assistant will deliver and assist clinical staff in the provisionof treatment, preventative care, health promotion and patient education.
Main Responsibilities:
ECG recording
Long term condition reviews
Assisting with Diabetes clinics
Spirometry subject to training
Injections under PSD
Testing of urine samples as requested by clinicians
Diabetic foot checks
Asthma reviews subject to training
NHS Health Checks
Blood Pressure Checks
Processing and management of laboratory samples requested by GPs/nurses
Vaccine/cold chain storage, monitoring and recording
Surgical equipment and vaccine re-stocking and stock rotation
Stock ordering, control and re-stocking consulting rooms
Clinical administration as necessary
Preparing and maintaining environments and equipment before, during andafter patient care interventions, including assisting GPs during theperformance of minor operations and contraceptive procedures
Assisting in the assessment and surveillance of patients health andwell-being
Undertaking specific clinical activities for named patients that havebeen delegated and taught specifically in relation to that individual
Helping to raise awareness of health and well-being and how it can bepromoted
Assisting with the collection and collation of data on needs related tohealth and well-being

Manage, review and identify learning fromcomplaints, incidents and near-miss events relating to the practice, teamand self.
Ensure compliance with policies, proceduresand guidelines for self and others, by acting or alerting management teamif the practice appears to contravene policy, or if there are concernsover any aspect of patient care.
CONFIDENTIALITY
Maintain confidentiality of information, acting within the terms ofthe Data Protection Act and Caldicott guidance on patient confidentialityat all times.
Maintain an awareness of the Freedom of Information Act.
Information relating to patients, carers, colleagues, otherhealthcare workers or the business of the practice may only be divulged toauthorised persons in accordance with the Practice policies and proceduresrelating to confidentiality and the protection of personal and sensitivedata
HEALTH& SAFETY
Comply with Practice health and safety policies by following agreedsafe working proceduresActively reporting of health and safety hazards and infectionhazards immediately when recognised.
Keeping work and general areas clean and tidy, and usingappropriate infection control procedures to keep work areas hygienic andsafe from contamination.
Correct personal use of Personal Protective Equipment (PPE) andensuring correct use of PPE by others, advising on appropriatecircumstances for use by clinicians, staff and patients.
Makingeffective use of training to update knowledge and skills
SAFEGUARDING
Thepost-holder is expected to be aware of adult and child safeguardingresponsibilities in the context of the post and to undertake necessarytraining, both at induction and on an ongoing refresher basis as appropriate.They will be familiar with Practice safeguarding policies for children, adults,domestic abuse and Prevent and be confident in raising any concerns through theappropriate channels.

Person Specification
Experience
* Experience of working in a primary care or health care environment.
* Experience of working with the general public.
* Wound care/removal of sutures and staples.
* ECGs, 24 hour BPs, venepuncture, LTC Reviews and Spirometry.
* SystmOne User Skills.
* Competent in the use of Office and Outlook.
* Willing to adapt and develop as the Practice requires.
Qualifications
* Healthcare qualification (Level 3 or 4) or working towards gaining equivalent level.
* Care Certificate.
