Main duties of the job

The role ofCancer Care Coordinator is to focus specifically on cancer care; to helpimprove the uptake of cancer screening programmes and patient experience byproviding personalised care planning and support through the cancer pathway andsupporting the delivery of cancer requirements. The Cancer Care Coordinator cansupport early detection and prevention and ensure the early part of the patientsjourney is as seamless as possible. They are also able to offersupport/signposting post diagnosis and support personalised care agenda. Theycan provide extra time and capacity to support patients in preparing forclinical conversations and work closely with GPs and other primary carecolleagues. They offer support and information to patients and theirfamilies/carers throughout the cancer journey and across the cancer pathway.

About us

Connexus Primary Care Network brings together five GP Practicesserving 50,000 patients in the Brislington, St Annes, Knowle, Totterdown andStockwood areas of South Bristol. ThePCN is located inSouth Bristol locality of BNSSG and employs a small staff team that isresponsible for delivering a range of services under contract with NHSEngland.


Job responsibilities

PRIMARYDUTIES AND AREAS OF RESPONSIBILITY:

To assist thePCN and its network Practices in delivering improvements to the services weprovide to our patients in relation to participation in national cancerscreening programmes. To support our patients accessing screening programmesand addressing any necessary barriers or inequalities to access as able.

Ensuringrobust and supportive referral practices are in place for patients suspected ofhaving cancer; including use of guidelines, professional development, andsafety netting of those referred. To offer personalised care and support topatients on their cancer journey as needed. Ensure FIT testing/full resultsfor suspected gastrointestinal cancer referrals are completed.

In support ofcollaborative working the post holder will form productive and supportiverelationships with PCN staff who have the skills, knowledge, and remit tocontribute to this work. e.g., non clinical cancer champions, socialprescribers, pharmacists, secretaries etc.

Take forward proactive monitoring and tracking of patientssuspected or confirmed of having a cancer diagnosis ensuring that their journeyis processed in a timely and efficient manner, in line with Cancer Waiting TimeTargets and provide adequate safety netting.

To support cancer care reviews, prepare patients if needed andsignpost to other services within the practice or PCN as needed to patients e.gMacmillan.

Provide advice and support to practices on cancer audit/referralreview of cancer diagnoses. To work with practices to collate the learning fromcase reviews to identify any trends or learning. This may include coordinatingpractice cancer meetings, attendance, minutes and sharing learning/outcomes.

Provide the practice with support to hostpeer-to-peer learning events that look at data and trends in diagnosis andscreening across a Network.

Create or update a Library of data packs and other resources, tosupport the delivery of information to patients in a wide variety of formats tomeet the needs of all patient groups, including those with LearningDisabilities, and people for whom English is not a first language.

Review practice coding for report buildingand templates to ensure consistency across the PCN and accuracy of data.Identify coding anomalies and liaise with Ardens (template and reports used byall member practices).

Provide administrative support for project evaluation andfeedback. To coordinate projects and project manage programmes that will ourpatients in relation to participation in national cancer screening or any newSWAG projects or new workstreams.

Supportcalls to patients with appropriate information so they are aware of what toexpect when referred (e.g. info available on WCA website/Macmillan)

follow up on watch and wait scenarioswhen patients symptoms do not meet the criteria for referral. This could bevery beneficial in vulnerable patient groups e.g learningdisability/Dementia/mental health.

Project support

Current projects supportpractices in uptake/DNA reviews for all screening programmesbreast/bowel/cervical/lung - Suggest run searches monthly and initiate processfor recall/offering of these programmes in the most appropriate format forpatients. This should have a focus on inequalities for access eg languagebarriers, digital exclusion, easy read formats, disabilities.

Community engagementevents working with our CIL to support the cancer/health promotion elements,liasing with VSOs, health promotion, stop smoking just as examples.

26/27 QOF/DES/LES contractualelements (awaiting full clarification)

DES 26/27 states - Cancerand non-cancer screening

o Requirements to improvecancer referrals, early diagnosis and screening uptake, including expectationson referral quality, stronger safetynetting and clearer responsibilities forsupporting eligible patients have been included. This will provide operationalclarity and support consistent, effective delivery across PCNs.


LOGISTICS

* Run Ardens searches twice weekly for new cancer diagnoses and 2wwreferrals for all 5 practices (unless a practice prefers in house)
* Offer support or review to new diagnoses method tailored toindividual/practice preferences
* Coordinate and compile patient review lists for clinicians toreview for 3 month cancer review meetings in all 5 practices, diarise and minute. Disseminate anonymised learning to teams if required.
* Search and review potential missed 2ww referrals in all 5practices and advice practice team accordingly. Flag DNAs or significantdelays. Ensure robust safety netting processes are in place for all practices
* Identify vulnerable/health inequalities in our cancer referralsand those at high risk (LD, SMI, drugs and alcohol) and offer support withvarious aspects which could include attendance and appropriate information in aformat suitable to them. If appropriate to involve colleagues eg socialprescribers, transport support to enable attendance.
* As above but for attendance to their cancer carereviews/appointments in practices.
* Coordinate the quarterly cancer care leads clinicians meeting,minute and disseminate learning/outcomes to practices.
* Reports/queries to the cancer leads in practice and PCN Cancerlead; informal supervision also provided via this route.
