An exciting opportunity has arisen to join our innovative and forward-thinking pharmacy team at Swansea Bay University Health Board. We are looking for a highly motivated and experienced Pharmacist to take a leading role in promoting safe, effective, and evidence-based medicines use across our Health Board as Medications Safety Officer.
As part of this role, you will contribute directly to patient care through our pharmacy-led VTE clinic, helping to improve patient outcomes and enhance the quality of care we provide.

Main duties of the job
The post holder will undertake clinic sessions at the pharmacy-led VTE clinic with responsibility for the assessment and prescribing of
initial anticoagulation and ongoing monitoring and management and will:
Undertake an assessment role within the VTE clinic, with responsibility for the ongoing assessment of patient’s symptomatic recovery from VTE.
To have scope to develop skills involved with the assessment and diagnosis of DVT in patients who have been referred to AGPU from their GPs.
Lead, develop, deliver and evaluate the medication safety programme for the Health Board.
Act as the designated Medication Safety Officer for the Health Board in line with the national directives.
Be the lead pharmacist advising the Health Board’s medicines safety programme and ensure it develops in line with the local, regional and national directives.
Work closely with the Health Board’s Patient Safety stakeholders to review and identify all medicine related incidents and their review and actions.
Act as pharmacy lead for the Medication Safety Group.
Ensure that medicines used are appropriate, safe and supported by high quality evidence.
Monitor processes to manage appropriate and safe medicines usage and expenditure.
Input into the development of and undertake medication safety audits in conjunction with acute and primary care.
Contribute to Medication Safety training within the Health Board.

Essential Qualifications & Knowledge
Essential criteria
1. Master’s degree in pharmacy (MPharm).
2. Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent experience.
3. Current and continued registration with the General Pharmaceutical Council (GPhC).
4. Independent Prescriber (or currently undertaking qualification).
Desirable criteria
5. Accredited Tutor for clinical pharmacy diploma.
6. Member of the RPS
Experience
Essential criteria
7. Experience of working in a range of clinical specialities.
8. Experience of contributing into the pharmaceutical care in specialist area.
9. Evidence of clinical audit, service improvement and implementation of procedures and protocols.
10. Evidence of Change Management Skills.
11. Evidence of promotion and evaluation of best practice. Ability to undertake autonomous practice.
12. Evidence of working with the multidisciplinary team. Professionally accountable for own actions.
Desirable criteria
13. Proven track record of competence
14. Evidence of clinical audit, practise research, implementation of procedures, protocols.
15. Lecture on specialist subject. Teaching experience. Experience of being a preregistration pharmacist tutor.
16. Experience of working a in primary care setting. Experience of working in Medicines Information.
