* Experienced Social Worker - PO3: £47,362 - £50,339

Overview
Join the Children with Disabilities Team in Telford and Wrekin to support children and young people aged 0 to 17 with complex health, medical and learning difficulties, and their families. The role involves assessing identified social care needs, developing holistic support plans, and working collaboratively with a multidisciplinary team to help children reach their potential.
Responsibilities
* Plan and review social care needs for children and young people with Special Educational Needs and Disabilities.
* Work directly with children, young people and families to listen to their views and ensure they are fully engaged in support planning.
* Engage in multi‑agency working with partners in health, education and social care.
* Contribute to safeguarding and welfare assessments and maintain statutory responsibilities.
* Provide reflective supervision and support to colleagues within the team.
Qualifications
* Qualified Social Worker with relevant experience (PO2 or PO3 level).
* Knowledge of statutory duties for children in need, children cared for by the local authority and child protection.
* Experience in working with children with disabilities is an advantage but not essential.
* Pass a current criminal records check and meet all regulatory requirements for working with children and vulnerable groups.
