Join to apply for the Triage and Assessment Practitioner role at Compass Bloom (MHST).
About The Role
You’ll be the first point of meaningful contact for children, young people and families accessing our service. Working with a supportive, multidisciplinary team, you’ll carry out holistic assessments to understand each young person’s needs and ensure they’re matched with the right support at the right time.
You’ll take the lead on triaging new referrals, conducting comprehensive assessments, and liaising with families, schools and professionals to develop effective care plans. You’ll also help manage the assessment rota and contribute to the ongoing development of efficient, inclusive pathways into care. Every decision you make will directly impact a young person’s experience of the service and their journey to better emotional wellbeing.
This role is perfect for someone who thrives on detail, values relationships, and can balance empathy with structure. If you’re passionate about early help and prevention, and want to be part of a service that puts children and young people first, we’d love to hear from you.
This opportunity is a 6‑month Fixed‑Term Contract.
