Responsibilities include, but are not limited to:

Requirements:
* Social Work Degree.
* At least two years post qualifying experience as a Social Worker, assessing and managing risks.
* Previous experience of working with older people and their families / carers.
* Proven ability to undertake assessment of older people and their families / carers.
* Valuing diversity and using positive terminology referring to service users.
* Commitment to a needs-led approach to maintaining maximum independence and offering opportunities for people to remain in their own homes.
