3 months contract with local authority This role involves conducting comprehensive assessments for individuals with permanent or complex disabilities within the community. The primary objective is to provide services that prevent the need for ongoing support, ensuring that environmental changes facilitate independence. As part of a multidisciplinary team, you will evaluate the effectiveness of current care provisions and explore ways to enhance user independence. The position also includes supervising Occupational Therapy Assistants and students, and collaborating with various stakeholders to maximize service users' independence. Responsibilities: Conduct assessments under relevant legislation, such as the Care Act and Housing Grants, Construction and Regeneration Act. Participate in a multidisciplinary team to review and improve care provisions. Supervise Occupational Therapy Assistants and students as needed. Deliver services aligned with Adult Social Care goals. Collaborate with service users, carers, and professionals to enhance independence. Review support packages, equipment, and adaptations. Maintain accurate and timely case notes. Assess and prescribe Telecare and assistive technologies. Provide equipment and adaptation recommendations. Work within legislative frameworks and policies. Support the neighbourhood model in Hackney with health and social care partners. Conduct functional assessments to aid social workers in care package development. Offer OT interventions, including advice, retraining, and equipment recommendations. Present specifications for environmental adaptations for disabled residents. Manage risks related to specialist equipment in home environments. Recognize and respond to vulnerable individuals with appropriate support. Assist with safeguarding adult enquiries and risk assessments. Provide information and advice or direct customers to universal services. Promote activities and services that enhance health and wellbeing. Ensure a strength-based, person-centred approach that promotes independence. Requirements Knowledge of relevant legislation, including The Care Act 2014 and Housing Grants, Construction and Regeneration Act 1996. Awareness of local political drivers influencing Hackney policy and practice. Experience working in a multidisciplinary environment. Ability to conduct comprehensive OT assessments for complex needs. Skills in developing and delivering support packages and interventions. Competence in managing risks associated with specialist equipment. Experience in safeguarding and risk assessment. Strong communication skills for reporting and collaboration. Commitment to professional development and adherence to RCOT standards.
