We are seeking to recruit an Urgent Response Support Worker to support adults returning to their own homes after a stay in hospital or a reablement unit. Successful candidates will be key members of a multi‑disciplinary team providing support to regain independence during periods of physical or psychological change.
Responsibilities
This role provides a high standard of support within a reablement ethos, promoting independence, improved quality of life, dignity, and choice. Responsibilities include delivering medical, nursing, or therapy‑related treatment programmes or social care interventions to enable patients to return to independent living.
Qualifications & Experience
* Previous experience working with people delivering care or support.
* Hold a UK Driving Licence and have access to a car.
* A qualification in health and social care is an advantage.
The role requires a flexible approach to work rotas as the service operates a 5‑day out of 7 rolling rota, including weekends and bank holidays, between 7:00 am and 10:00 pm.
