Key Responsibilities Provide person-centred support tailored to individual needs. Assist with daily living activities, including personal care where required. Support individuals in accessing community activities, appointments, and social opportunities. Encourage independence and promote wellbeing. Maintain accurate records and documentation. Work collaboratively with colleagues, families, and healthcare professionals. Follow safeguarding, health and safety, and company policies at all times.
