Occupational Therapist – North Somerset Council
Hybrid Role | Community, Office & Home Working | 37 Hours per Week

North Somerset Council is seeking four experienced Occupational Therapists to join our expanding Reablement Services:
* 2 x Occupational Therapists – Community Reablement (Hospital Discharge Support)
* 2 x Occupational Therapists – Bed-Based Reablement

About the Role
You will play a key part in delivering high-quality reablement interventions across the North Somerset area.
This hybrid role involves a mix of community visits, office-based work, and home working, so flexibility is essential.
Key Responsibilities:
* Conducting community visits (mileage included)
* Managing a full and varied caseload independently
* Completing functional assessments and delivering tailored reablement plans
* Working collaboratively within a multidisciplinary team
* Promoting independence and positive outcomes for service users
Contract Details:
* Contract Length: 6+ months, ongoing
* Hours: 37 hours per week
* Location: North Somerset (local candidates preferred due to travel requirements)

Essential Requirements:
* HCPC-registered Occupational Therapist
* Strong clinical reasoning skills with a focus on reablement
* Previous experience in social care reablement or intermediate care services
* Ability to manage a full caseload autonomously
* Commitment to promoting independence and achieving positive outcomes
* Flexibility to work across community, office and home-based settings
