Applicants who do not meet the essential criteria will be considered under the Annex 21 training programme, where whilst undergoing training, they will be paid 65% of the top of Band 7 and on completion of training will then move on to Band 7 scale.

The Role
- The Advanced Practitioners will work autonomously across Primary, Community and Acute Services during in and out of hours, covering seven days including public holidays. They will be responsible for clinical decision making for the population, including assessment and treatment of patients across the Primary, Community and Acute settings.
- The Advanced Practitioner works closely with the multidisciplinary team (MDT)/medical staff including the OOH Hub and Acute Services, communicating daily about any patient seen overnight or on weekend who needs to be followed up by the GP/Acute Services during normal working hours.
- The Advanced Practitioner provides assessment, clinical care and management of both stable and acutely unwell patients and their families, making complex, autonomous decisions using advanced skills, with overall responsibility for assessment of care needs and delivery of advanced care. Medical telehealth/telephone support is available when required.
The working pattern for this role is 12 hour shifts over a 7 day working week.
In this key role, you will:
- Autonomously assess, diagnose and treat individuals with a range of acute, non‑acute and chronic presenting medical conditions across the lifespan.
- Practice at all times within the Professional Codes of Conduct.
- Manage own time to meet the various demands of the role, including clinical, educational and administrative elements.
- Develop and actively participate in research by working collaboratively with multidisciplinary and academic partners and other HEIs.
What You'll Bring
- Registered Nurse / AHP / Pharmacist with valid NMC/HCPC / GPHC Registration.
- Educated to or working towards or operating at Masters Level.
- Post Graduate Diploma in Advanced Clinical Practice and expertise within specialty/area of practice that demonstrates the required breadth of knowledge to lead safety effectively and efficiently.
- Comprehensive knowledge of clinical guidelines and standards within the specialty.
- A driving licence is required; this must be a full UK/EU/EEA licence.
