You may also work with other local teams to provide occasional cross-cover where required, helping ensure continuity of care across the region.This is a full-time position, offering stability, variety, and ample scope for continued learning and professional development.Main ResponsibilitiesAs a Band 6 Continence Nurse, you will:Carry out comprehensive continence assessments, treatment planning and follow-up careWork collaboratively with community nursing teams, acute hospitals, AHPs and social care colleagues to deliver a coordinated, high-quality serviceManage and advise on complex continence cases, supporting the wider MDT to ensure seamless care pathwaysPromote best practice in continence management, adhering to local and national guidelinesSupport patient education on bladder and bowel health, empowering individuals to manage their conditions confidentlyMaintain accurate, timely clinical records and contribute to audit, service development and quality improvement initiativesProvide professional advice and support to colleagues, carers and patients to ensure safe, effective careParticipate in ongoing training, supervision and reflective practice to enhance your clinical expertiseThe Ideal CandidateWe're looking for someone who:Is a Registered Nurse (NMC) with at least one year post-registration experienceHas a genuine interest in continence care, long-term condition management or community nursingHolds a full UK driving licence and has access to a vehicle for workHas strong communication skills, a compassionate approach and the ability to work autonomously as well as part of a small teamIs committed to delivering high-quality, patient-centred care and contributing positively to service development
