Horizon Healthcare is offering a 12-month fixed-term maternity cover position, with the exciting possibility of a permanent post for the right candidate.
* No on-call commitment
* No Acute visits
* On-call Partner support throughout the whole day
* A Clinical Pharmacy team who undertake all prescription tasks and all medication related correspondence and patients with long-term conditions
* Mental Health Professionals on site, managing MH consultations

You will be joining a forward-thinking General Practice with motivated multi-disciplinary teams of administrative and healthcare professional colleagues. Horizon Healthcare operates a "Primary Care Consultant" model for our team of GPs, providing value to the individual and a learning environment with on-call partner support. Administrative duties are reduced by our in-house specialist teams. Horizon Healthcare has a list size of over 27,000 patients and offers competitive rates of pay based on experience and skill levels.

Job Description & Responsibilities
Main duties of the job
To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the GMS contract. The post-holder will adhere to the GMC standards for good medical practice, contribute to effective management of the practice, lead by example, and maintain a positive, collaborative relationship with the multidisciplinary team.
* The delivery of highly effective medical care to the entitled population
* The provision of services commensurate with the GMS contract
* Generic prescribing adhering to local and national guidance
* Effective management of long-term conditions
* Clinically examine and assess patient needs and plan clinical care, referring to other services as appropriate
* Prioritise health problems and intervene in complex, urgent or emergency situations
* Promote healthy lifestyles and self-care
* Administrative tasks: referrals, repeat prescriptions and related duties
* Review medications following policies and guidelines
* Support anticipatory care plans
* Telephone triage and duty doctor roles on a rotational basis
* Maintain accurate clinical records
* Work collaboratively with the team
* Adhere to best practice and contribute to quality initiatives
* Accept delegated responsibility for QOF areas
* Attend and contribute to practice meetings
* Support development and maintenance of practice governance, training, financial management and HR
* Adhere to safeguarding policies for both adults and children
* Ensure compliance with the appraisal process and revalidation
* Commit to CPD and training across the practice team
* Support medical student training
* Support partners in achieving strategic aims and managing income and expenditure
* Review and adhere to practice protocols and policies
In addition, there are ongoing responsibilities to encourage collaborative working and to be an integral part of the general practice team and wider network. There may be occasions to carry out other tasks based on workload and staffing levels.
In addition to the primary responsibilities, the salaried GP may be requested to
* Participate in practice audits as requested by the audit lead
* Participate in local initiatives to enhance service delivery and patient care
* Participate in the review of significant and near-miss events (e.g., root cause analysis)
* Participate in clinical governance processes
* Provide leadership and education for the multi-disciplinary team
Person Specification
Experience
Essential
* Experience of working in a primary care environment
* Experience of continuing professional development
* Experience of QOF and clinical audit
* General understanding of the GMS contract
Desirable
* Minimum of two years as a salaried GP
* Experience of medicines management
* Experience of CCG initiatives
Other requirements
* Flexibility to work outside of core office hours
* Disclosure Barring Service (DBS) check
* Occupational Health Clearance
* Project lead as required with CQC, CCG and QOF
Qualifications
* Qualified GP
* MRCGP
* Vocational Training Certificate or equivalent JCPTGP
* General Practitioner (Certificate of Completion of Training CCT)
* Full GMC Registration
* National Performers List registration
* Appropriate defence indemnity (MPS/MDU)
* Eligibility to practice in the UK independently
Clinical knowledge and other skills
* Outstanding clinical knowledge for an experienced GP
* Excellent communication skills (written and oral)
* Strong IT skills and competent in Office and Outlook
* Experience with SystmOne
* Beneficial time management and teamwork, with ability to work autonomously
* Problem solving and analytical skills
