We have an opportunity for Specialist Clinical Pharmaciststo join our pharmacy team. Main duties of the job
We pride ourselves on the quality of the pharmaceutical carewe provide for patients. The successful candidate will be expected to deliver ahigh standard of near-patient specialist pharmaceutical care to our patients.In addition, they will support the pharmacy team to deliver governance,education, training and service improvement to their specialist area. They willhelp shape the strategic direction of pharmacy by supporting the development ofinnovative new services. To achieve this we will support the successfulcandidate with mentoring and development opportunities including independentprescribing, RPS portfolio development and other further educationopportunities. Applicants need to be able to work autonomously, be a teamplayer whilst motivating others. Applicants currently undertaking a postgraduateClinical Diploma may be eligible for financial support to continue this withus. Pharmacists that have already completed a diploma will be supported toundertake an Independent Prescribing qualification.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities
Deputising for the Pharmacist Advanced for their area andin so doing will be required tosupervise a team of clinical pharmacists, pharmacy technicians andpharmacy support workers, ensuring the delivery of safe, effective andefficient clinical pharmacy and medicines management services to an agreedstandard and to a designated group of patient and service users
Providing medicines management, medicines optimisation andmedicines reconciliation services to help achieve department priorities andtargets on a designated ward or group of wards, day case units or clinics asdecided by the Lead Pharmacist
Helping to develop and implement local policies andguidelines on good prescribing practice.
Influencing local practice to ensure safe medication use,and to ensure that their own practice and the practice of others effectivelyacknowledges good prescribing practice, patient safety and risk minimisation.
Being a point of contact for nursing and clinical staff regardingmedicine management issues.
Providing medicines information to healthcareprofessionals and new drug reviews as appropriate utilising the range ofinformation resources available, including the library and the internet, and tomake an informed decision on the appropriate response required.
Being involved in providing clinical pharmacy service todesignated multidisciplinary team structures e.g. out patient clinics, preassessment.
Providing high qualitypatient education onmedicines use inboth in-patient and outpatient settings.
Engaging in activities relating to clinical effectivenessincluding: drug use review, clinical audit and clinical self-audit and tocontribute to ongoing measures of service evaluation.
Supporting the planning and implementation of servicechanges and new treatments, as agreed within the pharmacy service.
Acting as a professional checker in thedispensary and/or chemotherapy service
Person Specification
Qualifications
* Masters Degree in Pharmacy or equivalent Completed a suitable postgraduate qualification (eg Clinical diploma) or demonstrated sufficient experience to meet RPS post-registration foundation competence standards
* Evidence of CPD and education
* Independent Prescriber or willing to work towards
* Post qualification training or experience in the relevant subject area, preferably to Post graduate Masters level
Statutory Registration
* Registered as a Practising Pharmacist with the GPhC
* Membership of RPS or a relevant Clinical Pharmacy Organisation appropriate to the subject area
Experience
* Sufficient experience to be able to provide near-patient clinical pharmacy services
* Able to work as an autonomous practitioner in all areas of pharmacy practice
* Ability to build working relationships with other health care professionals
* Experience of delivery of formal education and training sessions Hospital pharmacy experience Experience within the relevant subject area/speciality

* Good understanding of current pharmacy /NHS issues
* Ability to demonstrate an area of expertise within a field of pharmacy practice
* Examples of clinical audit and involvement in service development Examples of development of evidence-based clinical guidelines and protocols
