The transitions planning team supports young people to prepare for adulthood by working closely with those important to them, along with statutory services covering health, education, children’s social care, and the voluntary sector. We recognize that preparation for adulthood involves celebration, change, choice, decision-making, and challenges for all young people.
We prefer candidates with previous experience or knowledge of working in Learning Disability roles. The position is available until the end of March 2024, and candidates should be available to start as soon as possible.
We operate under the statutory framework of the Care Act 2014, conducting assessments of need where it significantly benefits the young person prior to reaching adulthood.
Our goal is to provide seamless service planning and support, focusing on helping young people lead independent lives. We aim to work alongside families and professionals to empower young people to recognize and achieve their aspirations, increasing their choice and control over their lives, and improving their connection with the local community as they transition into adulthood.
We work with young people with learning disabilities, neurodivergence, long-term physical health conditions, sensory impairments, and those who are care experienced.
The role requires a qualified social worker with experience working with young people or adults with care and support needs, employing a person-centred and strengths-based approach. Experience working with young people with learning disabilities and senior/advanced practitioner experience is preferred. Candidates should have good legal literacy and working knowledge of the Care Act 2014, Children Act 1989/2004, Mental Capacity Act 2005, Children and Families Act 2014, Equality Act 2010, and Human Rights Act 1998, which underpin our practice.
