About the Role:

As a Specialist Social Worker within our High Needs Team, you’ll play a pivotal role in delivering comprehensive support to individuals grappling with mental health issues and learning disabilities. Your duties will involve assessing their needs, devising personalised care plans, and fostering collaborative relationships with fellow professionals and agencies to ensure the provision of top-notch services. This role encompasses direct client interaction, advocacy, and facilitating access to vital resources and support networks.

Key Responsibilities:

* Conducting assessments to ascertain the needs of individuals grappling with mental health issues and learning disabilities.
* Devising and implementing personalised care plans in collaboration with service users, their families, and interdisciplinary teams.
* Extending support and advocacy to empower individuals in accessing suitable services and resources.
* Regularly monitoring and reassessing the efficacy of interventions, making adjustments as required.
* Collaborating with external agencies and partners to orchestrate holistic support for service users.
* Maintaining meticulous, up-to-date records in accordance with legal and regulatory standards.

Requirements:

* Qualified Social Worker with registration with Social Work England.
* Proven experience in working with individuals facing mental health challenges and learning disabilities.
* Proficient in assessment, intervention, and care planning.
* Exceptional communication and interpersonal skills, coupled with the ability to establish rapport and instil confidence in service users and their families.
* Capable of working harmoniously within a multidisciplinary team and collaborating effectively with external stakeholders.
* Dedication to continuous professional growth and adherence to professional standards and ethics.
