Great opportunity for a Band 3 Capacity Coordinator
The Capacity Team is responsible for assisting in placing the Right Patient in the Right Place at the Right Time. They support the Emergency Department in providing timely emergency care by maximizing patient flow throughout the hospital.
The Capacity Coordinator will work alongside site practitioners to manage effective patient flow by identifying capacity within ward areas and facilitating timely discharges.

Key responsibilities include:
1. Maintaining a current and accurate bed state, including daily planned discharges, bookings, admissions, GP referrals, outliers, repatriation, additional capacity, critical care step-down, and discharges to community facilities.
2. Communicating trust escalation levels to all appropriate staff.
3. Collaborating with wards, ED, Discharge Team, and multidisciplinary teams to ensure timely discharges or transfers.
4. Escalating issues impacting patient flow to relevant managers.
5. Updating MDT databases for timely and accurate data reporting on bed utilization and other reports.
6. Supporting solutions to minimize potential breaches within the department, escalating as needed.
7. Liaising with ED, medical, and surgical assessment units to ensure smooth patient flow.
8. Keeping the nurse in charge of ED and AMU informed about bed availability.
9. Coordinating patient transfers and escalating delays or concerns.
10. Booking and tracking ambulances, liaising with WMAS and other transport agencies.
11. Supporting the implementation of the trust escalation policy.
12. Communicating effectively to mitigate complaints and concerns.
13. Liaising with GPs, AHPs, CCG, and social care partners as necessary.
14. Supporting the preparation of weekly operational plans and daily bed meetings.
