Clinical Lead Transfer of Care Hub

Responsibilities
- Facilitate timely and safe discharges/transfers from hospital.
- Provide effective communication with patients, carers, ward staff, multidisciplinary team and community partners to ensure well‑planned discharge.
- Identify and resolve critical delays in diagnostic, treatment and care processes.
- Plan and coordinate patient journeys from admission to discharge, prioritising care and managing a defined case load.
- Act as a resource for clinical areas related to discharge planning.
- Deputise for Complex Case Managers and support staff development in discharge planning.
- Lead multidisciplinary team discussions, family meetings and liaise with community partners for continuity of care.
- Provide advice on safe and appropriate discharge, especially for patients with complex needs.

Professional
- Registered professional.
- Health‑related degree or equivalent.
Experience
- Understanding of continuing healthcare.
- Experience of discharge planning.
Role Specific
- Knowledge of health services.
- Demonstrable high levels of communication.
- Ability to perform assessment, planning and implementation.
- Understanding of local and national policy on discharge.
