What you will do:
* Conduct social care assessments and devise timely care plans for hospital inpatients to ensure smooth discharge processes
* Perform mental capacity assessments, best interest decisions, and manage safeguarding responsibilities as needed
* Gain a thorough understanding of Section 117 and actively participate in meetings, ward rounds, and professional gatherings
* Join the duty rota, collaborating with colleagues to maintain consistent support and services
