A fantastic opportunity has arisen for a Band 6 Lead Mental Health Nurse or a Band 5 ready to take the next step to being a Band 6 nurse, to join the older adult In-Reach and Home Treatment Team in Derby. The primary goal of the service is to support people within their own homes, who are experiencing a breakdown of their mental health and to prevent admissions to hospital where appropriate.
Responsibilities:
* The post holder will be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.
* You will work collaboratively with colleagues, key partners, and those within the local community to develop services to meet the mental health needs of the community.
* The post holder will employ a wide range of skills, building on and developing their expertise and actively seeking advice and collaboration on issues beyond their competence.
* As a senior registered practitioner you are professionally and legally accountable under the NMC code of professional practice.
* Lead a shift, cover duty and formulate detailed assessments and person-centred care plans to assist in preventing hospital admissions where appropriate.
* Provide supervision to team members and mentor students.
* Work within a supportive MDT and maintain close links with the Community Mental Health Teams, GPs and social care.
Qualifications:
* Band 6 Lead Mental Health Nurse or Band 5 Mental Health Nurse with readiness to progress to Band 6.
* Experience working with older adults with functional, mental health illnesses.
* Registered with the NMC.
* Ability to work shifts, including Saturdays, Sundays and bank holidays on a rota basis.
