What You’ll Be Doing:
Join our team as a Pharmacist and play a key role in improving the lives of patients in rehabilitation and elderly care settings. In this role, you will:
* Perform comprehensive patient assessments
* Create and implement personalised care plans
* Deliver evidence-based treatments to optimise patient outcomes
* Collaborate with multidisciplinary teams to provide holistic care
* Maintain accurate patient records

Requirements & Qualifications:
To be successful in this role, you will need:
* A current and valid GPhC registration
* Prior hospital pharmacy experience (rehab, elderly care or acute wards preferred)
* Strong communication and interpersonal skills
* The ability to work independently and within a multidisciplinary team
* A commitment to delivering high-quality patient care

This role offers a Monday to Friday schedule with flexibility where possible. The contract is ongoing, with a strong possibility of extension based on performance and service needs.
