Role Overview
The Community Specialist Palliative Care position (St Helens and Knowsley) is part of a developing and forward‑thinking team that demonstrates high levels of patient and carer assessment and support. The role offers a full‑time Registered Nurse opportunity to develop competencies and skills in Palliative and End of Life Care.

Mentored induction and support from a Clinical Nurse Specialist in Palliative Care; a structured programme of learning and development.
Qualifications
* At least 3 years post‑registration experience.
* Excellent communication skills.
* Experience in Palliative and End of Life Care or Oncology is an advantage; experience caring for patients with long‑term or life‑limiting illness is sufficient.
Responsibilities
* Continuing responsibility for management of a caseload; ensuring continuity of care and acting as the lead person for communication to identified stakeholders.
* Providing proactive, responsive services that address anticipatory and maintenance care needs of patients/clients within the defined caseload.
* Conducting holistic assessments of care needs and delivering clinical care for patients/clients with acute and chronic highly complex needs.
* Practising autonomously and demonstrating evidence‑based clinical decision making.
* Working within guidelines to identify and manage risk and reporting identified risks to the team leader.
* Ensuring delivery of the quality strategy within the designated area, including establishment of systems and processes.
