The Short Term Assessment and Reablement Teams support people aged 18 and over in their own homes for a few days up to a few weeks. We provide a person-centred approach to help people regain independence, increase confidence, and learn new skills in all aspects of personal care and daily living.
Responsibilities
* Carry out first visits to introduce the service, complete initial assessments, and plan safe, effective reablement support.
* Assess home environments to identify risks and recommend equipment or adaptations.
* Support medication management, including completing MAR charts and reviewing medication needs.
* Provide advice and guidance to Reablement Workers and ensure safe working practices.
* Lead out‑of‑hours duty cover, offering problem‑solving support to staff and liaising with families, health partners, and other professionals.
* Support quality assurance, including accurate recording, incident reporting, and reviewing progress against goals.
* Work collaboratively with Occupational Therapists, Community Care Officers, Coordinators and Managers to deliver excellent outcomes for people.

Requirements
This role requires a full driving licence and access to a car. A taxi service will be provided where disability precludes this. The council reimburses mileage incurred for business use purposes.
