Senior Rehabilitation Assistants work with our Occupational Therapists, Physiotherapists and Reablement Coordinators to undertake assessments and provide review to ensure people are receiving the right level of input and / or re working on their outcome based goals.
Senior Rehabilitation Assistants work as part of a close team undertaking activities according to people's individual goal plans, encouraging them to do as much as possible for themselves, to live well and safely in their own homes. They also work with the Reablement coordinators to support and maintain quality standards of work delivered by the Rehabilitation Assistants.
Your role will involve completing assessments, determining risks and how to manage them, communicating and recording your feedback to others involved in people's rehabilitation.
You will need to be able to drive around the locality to work with people in their homes. Team working and communication skills are essential. You will be confident with handling IT systems to update written notes and electronic records.
