Overview
This is an exciting opportunity for a Social Worker to work as part of a Health and Social Care Integrated LD East Team.
Applicants should be able to demonstrate an awareness of current issues in relation to the Care Act, Mental Capacity Act and Care Co-ordination in particular.
Responsibilities
The successful applicant will have a varied role working with adult service users to complete initial assessments who present with varying levels of need and risks, physical and mental health needs and forensic histories.
Qualifications
* Dip.S.W., C.S.S. or C.Q.S.W. or equivalent and hold Social Work England registration
