Are you a people person who can motivate others and can support someone to work towards their goals? Being a Lead Support Worker is a rewarding and key role, helping older adults who have had an illness or injury to regain their independence at home.
The Joint Community Rehabilitation Service (JCR) is at the heart of care in the community. Our teams assist older adults after an illness or injury get back to independence in their own home. Working alongside other professionals, you will be working closely with clients to help them set the goals that will enable them to regain the skills and confidence they need to manage their everyday tasks. Your role will be varied and will include visiting clients in their own homes, teaching them new skills to help them regain independence and setting goals for clients and Support Workers to work towards, the progress of these goals will be reviewed weekly.
We are looking for people who:
* Have good communication skills.
* Have core values and ethics and the ability to empathise.
* Can work closely with health and social care professionals, including therapists within the JCR team.
* Are able to support people with specific activities, for example gait education and exercises.
* Are able to accurately record visits in report format, email and verbally.
Note: a driving licence and access to a car is required and the role involves working shifts between the hours of 7am and 10pm, including weekends and Bank Holidays.
