NMAHP Lead Advanced Frailty Practitioner – Band 8A

(Permanent Position, 37 Hours per Week)

NHSGGC one of the UK’s largest healthcare systems, is initiating a change programme aimed at improving urgent care. The newly established Interface Care Division will focus on integration between primary and secondary care and patient prioritisation. Over the next three years, the division will manage developments such as the FNC+Plus care model, the creation of a Virtual Hospital with 1,000 virtual beds and clinics, the introduction of Electronic Triage at hospital entrances, and the formation of Rapid Assessment & Care (RAaC) units to promote timely and efficient patient care. These measures are intended to bring care closer to patients' homes, optimise pathways, and reduce delays to ensure appropriate care provision.

We are seeking an NHMAHP Lead Advanced Frailty Practitioner (AFP) to work collaboratively with the Corporate NMAHP Advanced Practice Team. This role offers the opportunity to join the Advanced Frailty Practitioner team within the Flow Navigation Centre Plus (FNC+), at the core of the newly established Interface Directorate.

This position is evenly divided, comprising 18 hours as Lead AFP Clinical Practice at Interface FNC and QEUH, and 18 hours as Lead AFP Learning and Education.

The post-holder will work as part of the Advanced Frailty Practitioner Team in the Flow Navigation Centre, within the Interface Directorate (18 hours). Responsibilities include developing clinical skills and conducting virtual consultations with a range of patients through video or telephone calls. The role also involves delivering the Advanced Frailty Practitioner Learning and Education Programme, overseeing the related academic work-based learning module and portfolio development, and completing the necessary sign-off process (18 hours).

Applicants must be a NMAHP Advanced Frailty Practitioners who have successfully completed Advanced Frailty Practitioner training (including sign-off) and can demonstrate active engagement in educational initiatives. Up-to-date documentation of Continuing Professional Development and a demonstrated interest in contributing to AFP training are also required. In addition to extensive experience in Frailty, the Lead AFP will be able to demonstrate professional knowledge in Frailty working across unscheduled care and the delivery of patient flow and whole system solutions.

Applicants are required to be NMAHP Advanced Frailty Practitioners with a minimum of two years' experience as an AFP. Selection for these positions will take place through interviews.
