Job overview

Working across sites within ELHT NHS trust, you will be expected to deliver patient centred advice, support, and care for patients between the ages of 16 and 25 years old with diabetes within the secondary care service, their families, and carers. You will be expected to support all diabetes patients through all stages of their diabetes care.


Main duties of the job

In line with National Service Framework (NSF) and the Nationals Institute for Health and Care Excellence (NICE) guidelines you will be expected to provide advanced assessment of patients’ needs and advise on appropriate diabetes management. Additionally, you will be expected to support all diabetes patients through all stages of their diabetes care. The care of patients between 16 and 19 years of age is jointly provided by the paediatric and adult team, and you will be expected to work alongside both the adult and the paediatric team for these patients to provide a seamless transition of care into the young adult service at 19 years of age at which point paediatric input will cease. Your role will involve empowering patients with the skills and knowledge necessary to attain independence in self-management.

Detailed job description and main responsibilities

The post holder will support the management of transitional care, young adults and adults working alongside a highly skill team of consultants, specialist nurses in paediatrics, adults and specialist dieticians. In additional to your clinical role, you will aim to raise the awareness and improve standards of diabetes care through planning and implementation, of specialised programmes of education and training within your area of specialty. You will Act as an expert educational resource and support to all grades of staff in intermediate service, GP Practices and Community Nurses in the delivery of comprehensive diabetes care for transitional patients and young adults. You will manage your own caseload and participate in and, where appropriate, initiate research projects.

Essential criteria

* Registered Nurse Level 1
* Ability to prioritise and manage own workload.
* Car driver/car user essential
* Degree module in Diabetes management or equivalent experience


Desirable criteria

* Non-Medical Prescribing Qualification (willing to undertake)
* DAFNE/ DAP Education (Willing to undertake)
* Degree and evidence of study at master’s level (willing to undertake)
