The post holder will be the point of contact for all administrative issues relating to patients’ pathway of care. The post holder will work as part of a multi-disciplinary team and will provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer-focused, and knowledgeable point of contact.
Responsibilities include:
1. Processing and logging all referrals in line with Trust Access Policy and specialty SOP.
2. Scheduling new and follow-up appointments with the most appropriate clinician, ensuring capacity is used efficiently and escalating issues as required.
3. Ensuring diagnostics and investigations required as part of the patient pathway are requested, including those at other Trusts.
4. Assisting in coordinating appointments and procedures at other hospitals and organizations as needed.
5. Recording clinic outcomes accurately on the Trust Electronic patient record system (CRS), including off-site clinics.
6. Completing clinic cancellation forms under supervision and processing them according to SOP.
7. Rescheduling outpatient clinics as requested, in line with waiting time targets.
8. Using the Trust’s medical transcription system and ensuring letters are processed per SOP.
9. Liaising with patient records staff, ward clerks, and other organizations to ensure all medical notes, referral letters, results, and discharge summaries are available for outpatient appointments and consultations.
10. Supporting recognition of patients on cancer pathways and liaising with Cancer Services MDT coordinators for appropriate handover.
