Job purpose:
To contribute to the operational delivery of an effective Adult Social Care Service as part of an integrated team responsible for safeguarding, support planning, and promoting the wellbeing of adults with care and support needs, continuously improving outcomes.
Responsibilities include, but are not limited to:
1. Managing a complex and varied caseload with appropriate supervision, guidance, and support, in accordance with policy, procedures, guidance, and legislation.
2. Undertaking work within legislative frameworks, including preparing reports, statements, and representations for Court hearings and applications to the Court of Protection.
3. Communicating effectively in complex or high-risk situations.
4. Representing the department at multi-disciplinary forums and chairing meetings as required.
5. Making assessments and acting as a care navigator for individuals with eligible social care needs, in collaboration with health and other agencies.
6. Ensuring the delivery of services within financial resources to meet assessed needs and outcomes.
7. Collaborating with Social Care Assessors on tasks and work areas as agreed with the team leader or community manager.
Requirements:
1. Dip SW, BA, or MA in Social Work or equivalent, or expected qualification within the next 5 months.
2. Social Work England registration or expected registration within the next 5 months.
3. Knowledge of Community Care Legislation, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
4. Experience in completing assessments and support plans, and using IT for documentation.
5. Ability to research and incorporate new guidance into work effectively.
6. Excellent organizational skills to prioritize and manage caseloads, producing high-standard work within deadlines.
7. Enhanced DBS check within the last 12 months, with the possibility of fast-tracking through the recruitment process.
