Overview
The borough has three main bases for School Nursing aligned to Family Hubs and health centres. The teams are working towards a more integrated service offer. H&F form part of the Inner Northwest London (INW) division and a Clinical Service Unit (CSU) that includes Hammersmith and Fulham Health Visiting & School Nursing 0-19, Westminster Health Visiting, Kensington and Chelsea Health Visiting and an integrated 0-19 SPA. There are three teams in the borough split geographically (North, South and Central), managed by the School Health team lead. The borough has an experienced clinical lead overseeing Health Visiting and School Nursing with specialist health visitors covering perinatal mental health, SEND, MESCH and infant feeding. Each team has a team lead, qualified health visitors, community staff nurses, nursery nurses, a dedicated practice development nurse, and are aligned to early help staff. This skill mix supports integrated and progressive services. We focus on children, young people and families, and seek a School Health team lead to join our diverse teams. We champion community health professionals to deliver the best care closer to home and support families to stay well.
