Overview
Senior Pharmacist - Prescribing Support Team role at NHS Scotland. One year fixed-term post. This exciting role involves working with GPs and the primary care multidisciplinary teams to improve medicines management and deliver excellent patient care. The pharmacist will deliver the pharmacotherapy service as part of the GP contract and support patients with their medicines. The role is patient-facing, may involve taking on a prescribing role and developing an individual clinical caseload in line with Achieving Excellence strategy objectives. Applicants should be qualified independent prescribers or be prepared to work towards this qualification. You will be part of the NHS GGC primary care pharmacy team, with objectives to improve the quality and cost effectiveness of prescribing and medicines use for patients across NHS Greater Glasgow and Clyde.
You will work closely with GPs, other practice staff, community pharmacists, care home staff, members of the primary healthcare team and central team colleagues to develop and apply high quality, safe, cost effective, rational and evidence-based prescribing in the Health and Social Care Partnership (HSCP). You will work within GP practices, care homes and other healthcare settings to provide individualised clinical care to patients through clinics, to improve the safety and effectiveness of their medicines, prescribing where appropriate.
You should have sound clinical knowledge, good IT skills, excellent communication and interpersonal skills and be able to work autonomously as well as part of a team. You may work on joint projects/initiatives within the HSCP as required and promote awareness of prescribing management issues.
Responsibilities
* Collaborate with GPs and primary care multidisciplinary teams to improve medicines management and patient care.
* Deliver pharmacotherapy services as part of the GP contract and support patients with their medicines.
* Develop a prescribing caseload and provide individualised clinical care through clinics.
* Work within GP practices, care homes and other settings to improve safety and effectiveness of medicines.
* Contribute to projects to improve quality, safety, and cost-effectiveness of prescribing.
* Promote high quality, evidence-based prescribing within the HSCP and across teams.
* Travel between GP practices and care homes as required.
Minimum Requirements
* Masters Degree in Pharmacy or equivalent
* Registrant with the General Pharmaceutical Council
* Independent prescriber or working towards
* Demonstrable post-registration experience
* Must be registered with the General Pharmaceutical Council and able to demonstrate CPD commitment
* PVG Scheme Membership/Disclosure Scotland Check
* Experience of working with primary care teams and knowledge of GP systems is advantageous
