The role involves rotational shifts including evenings, weekends and bank holidays. We are open to discussing flexible working to promote work/life balance.
Responsibilities
* Support clients in the community across Manchester, encouraging engagement in drop-in activities at community centres, attendance at medical appointments, and participation in social inclusion activities.
* Work with clients in their own homes to support medication taking, activities of daily living, and provide emotional support; report concerns to the Outreach Manager or the Nurse Manager.
* Collaborate with qualified nurses as an Outreach Support Worker to recognise indicators of deteriorating mental health and help implement interventions.
* Promote independence, wellbeing and healthy life choices; assist with daily living skills following rehabilitation and support activities such as walks, theatre trips, gym sessions and building social networks in the community.
The Ideal Candidate
* Experience of working with people with long-term mental health issues and a good understanding of their challenges and the recovery model.
* Strong team player with energy, excellent communication skills, a flexible approach and a positive attitude to achieve targets in a changing environment.
* Empathy, understanding and patience; passion for helping people through conversation and support.
* Driving licence is highly desirable.
