We are looking for an enthusiastic Advanced (Nurse) Practitioner to join our team on the Frailty Same Day Emergency Care Unit
Our Care of the Elderly services have been developed extensively over the last few years and have a vision of providing high quality Comprehensive Geriatric Assessment for all patients who require it. To achieve this, we have developed a new service at the front door
The Acute Frailty Team are seeking to recruit a highly motivated individual as we continue to rapidly develop and expand our services. The ideal candidate will not only have an interest in improving patient care within frailty, but also actively contribute to the development of education and training. This is fundamental to the role as the upskilling of the local workforce builds awareness of frailty, leading to improved identification and management for this patient group.
Delivery of person-centred care, screening for frailty and comprehensive geriatric assessment (CGA) are just some of the interventions that the successful candidate will be involved in. You will be supported to develop your clinical practice and leadership attributes, working towards specialised frailty competencies within clinical practice. The role aims to facilitate early identification with focused interventions for all adults with frailty, presenting to our ED.

Main duties of the job
We can offer you a friendly team to work alongside as we deliver high quality end to end patient interventions. You will be supported to work
as an advanced autonomous practitioner, demonstrating a high level of practice and knowledge whilst participating in direct evidenced based patient care. He/she will be part of a multi-disciplinary team providing professional and clinical support in the care of patients.
To undertake health assessments; diagnosing, treating and discharging patients.
The Advanced Clinical Practitioner will provide expert care and treatment to patients within agreed protocols and clinical guidelines. However the Advanced Clinical Practitioner will also have the ability to make decisions outside these frameworks, where necessary based on best practice.
Provide support within the team and act as role model and resource, encouraging and developing a high standard of clinical skill and improving patient outcome.

Working for our organisation
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Qualifications
* RGN
* Masters
* None medical prescriber
* Relevant teaching qualification eg City & Guilds or Professional Trainers Certificate
* Registered Mentor
* Evidence of continuing professional development
* Experience of establishing effective communication
* Evidence of clinical competence and credibility in the speciality and relevant post-registration qualification experience which must be in an appropriate area of specialist practice at a senior level
* Exercise a high degree of autonomy within specialist area
