Permanent, full-time (37.5 hours per week)
Pay: £13.21 per hour (£25,838 per annum) and great benefits including Health Cash Plan

Following significant changes in the service, we are now recruiting for a Lead Support Worker, who will be at the heart of supporting customers transitioning from entrenched street homelessness to developing skills for independent living in the community.
You’ll make a big difference each day by helping our customers achieve their hopes and aspirations. Amazing, we know!
Typical day as a Support Coordinator (known as Lead Support Worker)
* Creating support plans with customers and coordinating a small team of Support Workers to implement these plans.
* Customer group: working-age adults requiring support to maintain their accommodation, move into more secure and permanent housing, and address secondary needs such as substance misuse, mental health, and physical health conditions.
* Assisting customers with daily activities like meal preparation, shopping, budgeting, maintaining tenancies, and moving to independent accommodation if able.
* Conducting risk assessments, support planning, goal setting, and regular reviews.

You bring
* Passion for supporting customers to live their best lives, with a collaborative approach and attention to detail.
* Experience in creating person-centered support plans and supporting colleagues in executing them.
* Experience in coordinating and assessing customer referrals.
* The ability to work independently, stay calm under pressure, and be resilient.
* Access to a car is helpful but not essential; we have a free staff parking facility.

Job details
* Flexible work pattern options are available, but you must be available for lates, earlies, and alternate weekends.
* We may need to adapt your start and finish times to meet customer needs.
* You will manage your own caseload and diary.
* Proficiency in technology for support plan creation, online learning, and collaboration is required.
* An Enhanced DBS check is required; we will cover the cost.
