What You Will Do:
Deliver high-quality case management for individuals with three or more complex needs, ensuring effective, person-centred outcomes.
Collaborate within a multidisciplinary team, developing and implementing tailored support plans.
Conduct home visits and community outreach, promoting engagement and sustainable progress.
Maintain accurate records and assess progress, adapting interventions where necessary.
Liaise with healthcare professionals, local services, and partner agencies to ensure a holistic and coordinated approach.
