Overview

This role is part of the integrated urgent triage team. It will be based in a modern open-space office with a multi-organisational integrated team (Paramedics, GPs, nurses, social workers, support staff, call handlers). The post involves 12-hour shifts (8am-8pm) and is predominantly non-patient facing, with regular opportunities to maintain clinical competencies. It includes involvement in Community Virtual Ward (CVW), Norfolk & Waveney Unscheduled Care Coordination Hub (UCCH), and related urgent care pathways as part of the system Single Point of Access (SPOC). You would join initially one service and progressively build skills to operate across all services in time.


Responsibilities

* To triage incoming referrals by evaluating the information received, determining priority of care, and allocating to the appropriate service with the necessary priority response.
* To document triage notes so that the patient’s record is complete and suitable for audit purposes.
* To escalate concerns by liaising with clinical teams in the community and the integrated team when appropriate.
* To assess, prescribe and arrange care interventions and assessments for all patients referred to the Single Point of Access (UCCH, 2h-Urgent Community Response, Rapid Response and Virtual Ward).
* To work within the integrated team to prevent unnecessary hospital admissions by utilising all available pathways.
* To use the remote monitoring facility for Virtual Ward patients, understanding escalation points and when further clinical support is required.
* To liaise with GPs, social services, inpatient teams, other allied health professionals, and the voluntary sector to ensure identified needs are met and care is coordinated appropriately.
* To ensure inputting into Cleo, SystmOne (or other patient records used by the service) is completed daily.
* To be accountable for work delegated to others within the team and to provide coverage as needed across VW and UCCH hubs.


Work Environment and Requirements

* Role includes exposure to bodily fluids and infectious material; universal precautions and infection control policies must be followed.
* This is predominantly non-patient facing, with regular opportunities to maintain clinical competencies.
* Be competent in all aspects of the VW Hub, UCCH hub, and UCR triaging and allocation within the scope of practice when assigned to those duties.
* Be able to operate across services as progressions allow, with a focus on escalation management and integrated care coordination.
