This role has been advertised by Hartlepool & Stockton Health on behalf of our member practice Densham Surgery. This exciting new role will provide safe, high-quality primary care nursing services to the practice population. Working within the NMC Code of Conduct, the post holder will work as part of the nursing team, using previous primary care experience to provide nurse interventions.
Main duties of the job
1. Deliver general clinical care.
2. Carry out nursing interventions and measurements for patients using clinical judgement.
3. Administer medications by injection, with the appropriate legal authorisation.
4. Recognise health promotion opportunities and give brief, focused lifestyle advice using motivational interviewing techniques, which may include brief intervention sign-posting.
5. Undertake initial assessment of patients presenting with wounds, produce treatment plans, and refer to other services as necessary.
6. Measure circulation by recording Ankle Brachial Pressure Index and applying compression therapy to patients with venous leg ulcers.
7. Remove wound closure materials including sutures, clips, and staples.
8. Take samples for the cervical screening programme according to recognised standards.
9. Administer vaccines to adults and children according to the UK immunisation schedule.
10. Administer prescribed therapies such as vitamin B12 and Depo medications, recognising side effects and contraindications.
11. Provide sexual health services.
12. Advise women on the suitability of oral and long-term contraceptive methods and be aware of local referral pathways.
13. Provide a nurse-run asthma, diabetes, COPD service to patients, including screening, diagnosis, ongoing management, and referral to other agencies in line with NICE guidelines and Quality and Outcome Framework indicators.

In addition to the main duties listed above, the role requires quality assurance and service support by participating in quality improvement and innovation, e.g., audit, significant event analysis, and development of new protocols and services. Recognise opportunities to reduce inequalities and unwarranted variations in healthcare for the practice population. Undertake mentorship for preceptor programmes and contribute to revalidation needs of nurse colleagues. Analyse data and develop initiatives to improve uptake of national screening and immunisation programmes, including coordinating new vaccination campaigns. Complete statutory, mandatory, and skills-specific training as defined by the practice. Participate in annual performance reviews, maintaining a record of personal and professional development, including NMC revalidation requirements. Take responsibility for your own development, learning, and performance. Identify gaps in knowledge and skills, and pursue further learning to enhance nursing service delivery.
