An exciting opportunity has arisen for a Substantive Consultant Urologist in Mid Yorkshire Teaching NHS Trust and Urology area network with Barnsley Foundation Trust. This is from the expansion of services provided across both Trusts.

The post represent an exciting opportunity to join a group of Consultant Urologists in providing state of the art urological services to a population approaching 1 million. In a year the Urology department will see 27,500 outpatients and undertake 7,700 elective and 4000 non-elective procedures. The philosophy of the department is based on the active promotion of sub-specialisation whilst supporting the needs of general urology in the context of a large district general hospital. The present post holders have a strong track record in terms of research, teaching and remain committed to maintaining the high standards that underpin the excellent reputation of the urology department. The department has a track record in innovation and change in practice recognised by GIRFT as in the top tier of urological centres nationally. We have an established robotic service with plans to expand this further, across our sites. We currently have two Da vinci systems on two sites, with plans to expand these further. There are urology investigation units on all four sites.

It is anticipated that the successful candidate will have a major interest in Robotic Pelvic Surgery and bladder surgery.

Main duties of the job

Sub-specialist roles in the robotic and endourological management of pelvic cancers would be ideal initially, however as a department we offer the full range of sub-specialty practice, therefore we may be able to accommodate alternative interests in time. We have a longstanding history in mentorship and will support the development of this pelvic cancer surgical role if the applicant so requests.

Urgent and Emergency Care duties including out-of-hours on-call for adults and children schedules will take account of the provisions of working time regulations and safe working

Out of hours on-call duties are currently shared with 10 other colleagues on a 1:13 basis {Monday to Thursday and the following Friday to Sunday}. 2.0 PAs are given for acute activity which includes an allocation for overnight on calls shared between 12 people.

Supporting Professional Activities in accordance with the Trust Framework, with at least a minimum of 1.5 SPAs included in the core 10PA timetable. Additional SPA or DCC time may be commissioned in line with the skills of the successful candidate and changing requirements of the service.

The successful applicant will work alongside Mr Dooldeniya and Mr Turo, who currently provide the minimally invasive urological pelvic cancer service.

THE DEPARTMENT

Departmental facilities are excellent. While major elective and emergency urological surgery is carried out in Pinderfields Hospital, outpatient work, day case surgery and urological investigations sessions are run on all of the UAN hospital sites. Urological investigations units are present on all four hospital sites and provide a focus for the diagnostic investigations and ambulatory treatments that patients may require. In patient care is provided through a 34 bedded urology ward at Pinderfields. Ward 14 at Dewsbury District Hospital and ward 36 at Barnsley Foundation Trust also enable inpatient stays for patients having less complex urological procedures. We have provision for day case and 23 hour stay on all sites.

The Department of Urology has been designated as a cancer team for major pelvic and upper tract cancer surgery within the regions Cancer Network. Cancer team designation has allowed development of the uro-oncology service. The oncology service is supported by a team of specialist nurses, MDT and cancer-tracking administrative support and a departmental research team who have an excellent track record of supporting recruitment into clinical trials.

The provision of advanced endourology, 3D laparoscopic surgery and robotic surgery is well established for both benign and malignant conditions affecting the upper and lower urinary tract. We have purchased a second Intuitive Da Vinci system and have installed it on the Dewsbury site. This is with plans to increase use at this site with expansion of the urological procedures to be offered robotically.

The functional service is supported by a tech led urodynamics service at both Trusts, as well as plans to expand the role of a nurse led review service to help bolster the nationally recognised spinal injuries service. There is a regional run MDT for discussion of complex cases. and a close working relationship with gynaecological colleagues.

The care of patients who are admitted acutely under urology is carried out with a high level of consultant input. The on-call consultant is free of routine duties and is able to concentrate their efforts on looking after patients who have been admitted acutely, running a rapid access (Hot) clinic, undertaking acute operating and training junior staff. We have had excellent GMC feedback based on the training culture that has been long established within the department

The Getting it Right First time (GIRFT) national report on urology has recommended the establishment of urology area networks to optimise quality and efficiency. Towards this aim, the urology department at Pinderfields has for the last 5 years successfully supported the service at Barnsley Hospital Foundation Trust and provides a tertiary referral service for specialist work. This has created a Urology Area Network.

Urological services are provided within the context of a multi-disciplinary team. There is excellent medical support from colleagues in Anaesthetics, Colo-rectal Surgery, Critical Care, Oncology, Palliative Care, Pathology, Radiology and Rehabilitation medicine.

It is well-recognised that applicants are unlikely to possess sufficient experience in this specialised field to enable them to take on the role without some assistance. There is therefore a commitment to provide the successful applicant with a package of support that will facilitate a smooth transition into this consultant role. This includes surgical mentorship for complex procedures (both open, laparoscopic or robotic dependent on the candidates experience). Applicants are invited to discuss transitional arrangements with the consultant team at an early stage. Experience

Essential

* Sufficient experience at senior level to be placed on the specialist register
* Wide experience in general urology

Desirable

* Computer literate
* Specific educational interests and evidence of educational ideas or project work.
* Broad experience in a laparoscopic / robotic experience in pelvic cancer
* Laparoscopic/robotic fellowship experience
* Broad urology experience e.g. TURBTs

Qualifications
