About the Role
Two casual positions available:
The HART service is to provides several care & Support offers under its registration as a domiciliary care service. This includes, reablement, which helps people return home from hospital, or prevent them from being admitted to hospital, and regain their independence. Homecare support to facilitate the timely discharge from acute and community hospitals to better understand the needs of the person once they have been able to return home. Short term crisis support, for those experiencing an urgent social care need. These service offers are all short-term support, helping someone to become as independent as is possible, and to ensure that the right support pathway is in place to meet any ongoing care and support needs
The purpose of the job is to:
1. To provide an overnight (10.30pm to 7:30am) triage and response service for those contacting the service as necessary.
2. Provide short term support for people with disabilities and those who are frail and recovering from illness, injury or to meet the needs of people in emergency situations, focusing on helping people to restore their optimum level of independence.
3. To undertake risk assessments and first visits to individuals who use the service.
4. To provide an overnight (10.30pm to 7:30am) triage and response service for those contacting the service as necessary
The Key Duties:
To act as the main contact point for the service between the hours of 10.30pm and 7.30am. Taking urgent referrals and providing direct support as is necessary following an assessment of risk.
To be able to provide direct service user support as necessary, including the provision of personal care, moving and handling and the administration of medicines and complete Risk assessment/welcome visit in the community. To respond to any emergency situations, by taking the appropriate action. contacting a doctor or the ambulance service etc. To identify, record and report all incidents / accidents, potential safeguarding and any other high-risk situations where a Service User’s safety may be at risk.
To undertake a range of delegated health care tasks as required, including prompting or the administration of prescribed medication support, the application of prescribed creams, eye and ear drops, catheter care, prompting exercises as prescribed by the physiotherapist. To complete and maintain accurate records as is necessary in respect of these tasks.
To maintain a high standard of administrative procedures by adhering to systems, completing accurate and comprehensive recording / documentation that evidences delivery of a reablement care and support service.
Special Factors
The nature of the work may involve the jobholder carrying out work outside of normal working hours. Ability and willingness to travel and work from various locations throughout the County and travel between sites as required
5. The postholder will be required to work shifts between 10pm and 7am and alternate weekends
6. This post is eligible for a DBS check under the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (. it involves certain activities in relation to children and/or adults) and is defined as regulated activity under Part 1 of the Safeguarding Vulnerable Groups Act 2006. Therefore, a DBS enhanced check for a regulated activity (includes a barred list check) is an essential requirement
About You:
7. NVQ level 2 in a relevant field or other equivalent qualification in a relevant subject or equivalent level of relevant experience working with adults in a social care setting.
8. Experience of working with other agencies and/or working in a multi-agency environment to identify and meet need with creative and pragmatic solutions.
9. Knowledge of work with people from minority cultures or groups. Commitment to equal opportunities and anti-discrimination practices and a desire to provide good quality services within allocated resources.
10. Ability to assist with moving and handling of people who use the service.
11. Ability to communicate clearly and accurately, both orally and in writing, with a wide range of people, including representatives from other agencies, using skills of negotiation and persuasion
12. The ability to prioritise effectively across a wide range of competing demands to meet deadlines.
13. An understanding of the Council’s Adult Social Care strategies, relevant regulations/ legislation and service frameworks underpinning practice in community care and residential settings
Other Requirements
Ability to work flexible hours on a rota basis between 10.30pm and 7.30am 365 days a year rota, including weekends, evenings and Bank Holidays.
Must have access to a vehicle for work at all times.
