Job Description
Do you want to help people access the care and support that they need? If the answer is yes, then working as a Health and Social Care Service Advisor in Health and Social Care Connect may be the role for you.
This role provides a valuable service to the community, to health and social care professionals and partner organisations such as the emergency services. It involves taking calls and responding to e‑mails from members of the public and a range of professionals such as GPs, and providing them with information and advice, or signposting or referring them onto the appropriate services to meet their health and social care needs.
This is an office‑based role, and staff work shifts on a rota covering the hours of 8am to 10pm including weekends and bank holidays. Whilst this is a 24/7 service, advisors are not expected to work every evening or weekend and there are pay enhancements for weekend working. Night shifts are not required in this role.
We will provide comprehensive training and a supportive environment in which to gain the knowledge of the range of health and social care support and services that are available for adults in the community and how these can be accessed by, and for the residents of East Sussex.
