What you can expect to be doing:
The Quality, Contracts and Purchasing Service pulls together the key elements of contract management and oversight of care and support services in Dorset and makes sure that these are matched to people who have assessed Care Act needs.
Working with our pre-commissioned services and hospital discharge teams, you will be responsible for sourcing care for individuals who are ready to be discharged from hospital, but need some support to maximise their potential to regain their fitness and return to their original baseline. At the other end of the scale you will also be preventing admissions to hospital by deploying these services to people that have become unwell in the community, which without timely intervention and support may result in a hospital admission.
The primary role of the team is to source care quickly and efficiently that meets the person's needs whilst maintaining flow through the hospitals and utilised services.
About you:
We are looking for strong, confident communicators who are able to deal with sometimes contentious and emotionally charged phone calls with service users and their family. You must be able to manage and prioritise your own workload and time in a pressurised environment and with a case management approach, ensuring time critical tasks are not overlooked.
You'll be a creative problem solver and critical thinker who has a good eye for detail to ensure all data is captured accurately, and will be able to multitask and switch between tasks with ease.
