Working for South Norfolk and Broadland Councils you will be delivering a discharge facilitation and admission prevention service in our well- established District Direct Team. This role requires quick and practical thinking and an ability to prioritise a rapidly changing workload.

Norfolk and Waveney have a well- developed Integrated Care System in which the NHS, Local Authorities and partners from the VCSE sector work together to maximise health outcomes. The District Direct Team which you would be joining is well-established with a supportive team of 15 colleagues. Training, support and practice supervision are always available.
What you'll be doing…
Taking referrals from District Direct Officers and conducting domiciliary assessments.
Liaising with a range of contractors to ensure homes are fit for discharge.
Working as part of the Discharge Hub, visiting patients' properties and feeding back as to works required to facilitate discharge.
Maintaining clear and accurate records of your interactions.About You
You must hold a full driving licence and have access to a vehicle, as travel is essential for this role. We are looking for someone with experience working with people in distress and a solid understanding of common health problems. Excellent verbal and written communication skills are key, along with the ability to build and maintain professional relationships. Personal resilience and a good sense of humour are vital qualities to thrive in this challenging yet rewarding position.
