The Emergency Department at Nottingham University Hospitals NHS Trust is recruiting to our CESR (Certificate of Eligibility for Specialist Registration) Fellowship Training Programme. We invite a motivated doctor to join our dynamic and friendly Emergency Medicine team. Our CESR workforce currently has 5 fellows, and we are looking to increase the team.
Depending on your level, upon commencement you may begin on our Tier 3 rota and, in your first year, aim to experience both adult and paediatric ED. After 12 months and a review of your achievements with the CESR Lead (including assessing your capacity to work as a senior decision maker), we can progress you to the Tier 4 rota and start planning placements in other specialties. If you have achieved your IAC (anaesthesia rotation) elsewhere in the NHS and have been working in a Senior Clinical Fellow capacity, we can consider commencement on the Tier 4 rota (registrar rota). Initially, we offer a 1-year contract, with a review after this period and potential extension for a further 2 years and entry onto the CESR pathway. Previous applicants need not apply.
Main duties of the job
We would employ you as a Junior or Senior Clinical Fellow for the first year to allow you to settle in and acclimatise to the busy department. Towards the end of this period, after review and assessments, if mutually agreed you may move into the CESR programme.
The CESR programme is expected to take around 4 years to complete. During this time you will work through competencies to gain the necessary skills and knowledge and develop a portfolio of evidence to meet the Emergency Medicine CCT curriculum. Rotations will be allocated through Acute Medicine, Intensive Care, Anaesthesia and Paediatric Emergency Medicine to enable you to acquire specialist knowledge and skills in these areas. In the Emergency Department you will work under the clinical direction of EM Consultants who are present in the department 24/7. This service is based at the QMC campus of NUH with a 24/7 roster managing adult and paediatric attendances.

QMC houses the Emergency Department with co-located adult and children\'s units. NUH is the Major Trauma Centre for the East Midlands. Our forward-thinking, research-active Emergency Department is one of the busiest in Europe with approximately 195,000 new attendances annually. The Children\'s ED sees 44,000 children and young people and has expanded its resuscitation capability with major capital investment.

Job responsibilities
Responsibilities and duties are set out in the job description and person specification. The post would suit those pursuing a non-consultant career grade post in Emergency Medicine, or an experienced Clinical Fellow/Registrar seeking to consolidate experience in a Major Trauma Centre.
* You MUST have completed MRCEM plus have IAC sign off to be eligible for a Tier 4 Senior role OR full MRCEM to be considered for Tier 3 Junior role
* Hold valid ALS and ATLS life support qualifications (in date)
* You will have at least 5 years post MBBS (or equivalent)
* At least three years working in an ED in the NHS at CT3 (or above) level is essential
Upon entering the CESR role, the department will support the post holders with their Specialist Registration application and provide planned secondments to Acute Medicine, Intensive Care Medicine, Anaesthesia and Paediatric Emergency Medicine (in no particular order). The department has 24-hour Consultant cover, and recent workforce investments have increased capacity across the clinical establishment.
The NUH CESR programme has run for 6 years and is highly successful, with graduates progress to Consultant within the trust or to substantive roles at NUH.
Responsibilities include management of the ED shop floor, adherence to the four-hour target, and continuous prioritisation of tasks; assessment and treatment of ED patients; liaison with appropriate specialties; acting as resuscitation team leader for seriously ill or injured patients (including children); liaising with the shop floor consultant in charge; working with the wider multi-professional team to ensure safe, timely, high-quality care in line with National Emergency Care Standards; supervision and training of junior doctors, Advanced Practitioners, Physicians Associates and Emergency Nurse Practitioners; and engagement in teaching, audits and quality improvement activities.
What we can offer you as a CESR Fellow in ED: monthly teaching, workplace-based assessments, shop floor supervision, opportunities to teach medical students and SHOs, audit and QIPs, acting up toward Consultant level, and preparation for Consultant interview and administrative duties.
Qualifications and Experience
Training and Qualifications
* MBBS or equivalent medical qualification
* GMC Registration (including current licence to practise)
* ALS and ATLS provider
* MRCEM
* ALS Instructor
* IAC sign off (for senior role)
Experience
* Completion of Foundation training (or equivalent)
* At least five years post MBBS (or equivalent)
* At least three years working in ED in the NHS at CT3 (or higher) level
* Experience in a large acute hospital (more than 1000 beds)
* Experience leading junior medical colleagues in the NHS
Planning and Organisational Skills
* Evidence of active involvement in design and running of clinical audits
* Leading or involvement in process changes with demonstrated delegation skills
* Experience presenting case reports, audits or research results
Communication and Relationship Skills
* Ability to cope with stressful situations and take responsibility
* Demonstrable spoken English to communicate effectively about medical topics with patients and colleagues
Analytical and Judgement Skills
* Understanding of clinical governance, capacity, consent, and duty of candour
Planning and Organisation Skills (cont.)
* Ability to make an initial management plan for an unwell patient

Salary: £52,656 to £65,048 per year. For JCF the salary is £52,656; for SCF it is £65,048.
