The Urgent Community Response (UCR) Service provides clinical interventions (including intravenous therapy) as part of a multidisciplinary team comprising Medics, Nurses, Paramedics, Physio and Occupational Therapists for admission avoidance pathways (2 hour response and general virtual wards), in line with the national specification for UCR and general virtual wards.

As a Health and Therapy Assistant the post holder is to work across the disciplines of nursing, physiotherapy, and occupational therapy, carrying out assigned tasks involving direct care to patients as part of the community based multi-disciplinary team. This role will involve working with patients in their homes within the policies, procedures and philosophy of Sussex Community NHS Foundation Trust, in support of and supervised by registered nurses and therapies staff.

Main duties of the job

As a Health and Therapy Assistant, you\'ll play a key part in helping people across Sussex live more independently after illness or injury. Each day will bring variety -- supporting individuals with personal care, therapy exercises, and health monitoring as they work towards their recovery goals.

You\'ll work closely with our nurses, therapists, and paramedics to deliver safe, person-centred care in line with each individual\'s personalised plan. This will include taking observations, assisting with wound care, supporting rehabilitation programmes, helping with medication, venepuncture, and ensuring therapy and mobility equipment is used safely and effectively.

You\'ll observe and record people\'s progress, sharing updates with your team so that care remains responsive and effective. Most importantly, you\'ll encourage, motivate, and empower people to build confidence and regain independence in their daily lives.

We\'ll support you every step of the way -- providing full training, the opportunity to complete your Care Certificate, and ongoing development through our competency framework. This role offers an excellent foundation to grow your skills and open pathways into nursing, therapy, and other healthcare professions within Sussex Community NHS Foundation Trust.

Person Specification


Qualifications

* General standard of education to GCSE level or equivalent
* NVQ level 2 or equivalent (in health and social care applicable subject), or be willing to undertake


Experience

* Evidence of consistent pattern of learning from education, training and experience


Skills

* Able to use recognised IT packages e.g. Microsoft Office including Excel
* Able to use of IT systems and keyboard skills including accurate data recording skills


Other

* Car driver (Valid Driving License for use in the UK)
* Access to a vehicle for work purposes
